Field Service Call Authorization(Required)

I have requested a field service call from Construction Edge Equipment and am responsible for the charges associated with this service

Credit Card Authorization(Required)

I authorize Construction Edge Equipment LLC. to charge my credit card for the initial $400 to schedule and dispatch a field service call and I authorize Construction Edge Equipment LLC to charge the balance of my field service ticket at completion of the repair.

Warranty Acknowledgment(Required)

I understand that field service call charges are not covered by my manufacturer's warranty and that I am responsible for those charges and I agree to pay in full these charges.

Repairs Not Covered(Required)

I understand that my repairs may not be covered under warranty.

Additional Trips(Required)

I understand that my repair may require additional trips accruing additional charges.

Service Estimate(Required)

I understand that a service estimate is an estimated amount, and charges may increase or decrease beyond the original estimated amount.

Failure to Pay(Required)

I understand that if I fail to pay my service bill in full that a mechanics lien will be filed with the state against my equipment.
